@Claudia: You need to first narrow down the variations of the long-tail keywords to the ones you want to rank for. You will never be able to go after or rank for all variations. Remember to have a goal for the keyword, besides just ranking for it. For example, your goal is to rank number one "cosmetic surgeon," but what do you want to do with the visits that you will generate from that keyword? 
 
@Lonnie: Add links to the text on your website pages that link to related website pages. Please download the slides because they will show you an example with each step.

Not actually sure how a link is established-I assume by them inserting my URL somewhere in their website? 
 
Be good to show each of these steps in your actual page properties sheet with real examples.

How does one manage long tail keywords for a website that has many products and services (i.e. cosmetic surgeon). Not only does keyword grader limit the # of keywords, long tail variations are endless.  
 
Any tips you can offer in this regard? Thanks.

You make an intriguing point that in selecting long tail keywords, that there be a specific goal for it, 'what i want to do with the visits I receive for it'.  
 
I can see a lot of work before me going through the long-tail keywords in the keyword grader and really cleaning that up.
